Cognitive Dysfunctions Observed in Schizophrenia and Evaluation of this: A Review

Since the days of Kraepelin, many authors have tried to understand local cerebral lesions and dysfunctions characteristic of schizophrenic patients. Although some authors using neuropsy-chological tests have reported larger left hemispheric deficit, another group of researchers have reported more diffuse and non spesific variations seen in neurocognitive dysfunction. As a result of neuropsychological and imaging studies performed on animals and humans, it was though that attention deficits may be related to increased subcortical mesolimbic dopaminergic activity in connection with functionally reciprocal connections with the frontal lobe. Although there are various other tests demonstrating frontal lobe damage, the WCST is the only test known to be spesific to the dorsolateral prefrontal area. There is ample literature suggesting that the Wisconsin Card Sorting Test measures conceptual thinking and high level problem solving ability.

Şizofrenide Gözlenen Bilişsel Bozukluklar ve Değerlendirilmesi: Bir Gözden Geçirme

Kraepelin zamanından bu yana araştırmacılar, şizofrenili hastalardaki özgül, bölgesel beyin lezyonlarmı ve işlevsel bozukluğu anlamaya çalışmışlardır. Nöropsikolojik testleri kullanan araştırmacılardan bazıları şizofrenik hastalarda göreceli olarak daha büyük sol hemisferik deflsit olduğunu bildirirken, diğer bir grup araştırmacı ise nörobilişsel işlev bozukluğuna özgü, daha yaygın ve özgül olmayan değişiklikler olduğundan söz ederler. Nöropsikolojik ve görüntüleme yöntemleri ile insanlarda ve deneysel amaçlı olarak hayvanlarda yapılan çalışmalar, şizofrenide görülen dikkate ait bozuklukların, frontal lob ile işlevsel olarak karşılıklı bağlantıları nedeniyle subkortikal mezolimbik dopamin etkisinde artış ile ilişkili olabileceğini göstermiştir. Wisconsin Kart Eşleme Testi (WKET) (Wisconsin Card Sorting Test), frontal lob hasarını gösteren başka testler de olmakla birlikte, özellikle dorsolateral prefrontal bölgeye özgül hasarı gösterdiği bilinen tek testtir. Nöropsikolojik testler sınıflamasında kavramsallaştırma ya da "yönetici" (executive) işlevi gösterdiğini, yüksek düzeyli problem çözebilme yetisini gösterdiğini belirten yayınlar vardır.
